To schedule an appointment, call 1-888-992-0019. We have the front desk to schedule your appointment on most weekdays. If no one answers your phone, you leave us a message with your phone number on our voice mail.

We recommend calling us a few days or a week before you need to be seen. Follow up appointments should be made as soon as possible to make sure you have an appointment time most convenient for you. As always, if you need urgent care during office hours, give us a call and we will do our best to have you seen that day.

If you are a new patient, be sure to fill out the patient information form found in the Patient form section of our website, this will save you time in the office.

The information you need to bring to my appointment:

Please arrive at least 5 minutes before your scheduled appointment. New patients should arrive at least 10 minutes earlier. Be sure to bring your current insurance card with you to each appointment. If you have the designated clinic or physician name on your insurance card, this means you need a referral. Please note that your payment will be due at the time of your appointment. Please notify the front desk of any insurance, phone number or address changes.

If you need to change or cancel an appointment, please call the office in advance, when possible. If you do not contact us regarding a cancellation, we charge a cancellation fee of $25.

Please bring the following to your scheduled appointment:
  • Your insurance card
  • A list of current prescription and/or over-the-counter medications you are taking (please include your dose and frequency)
  • Pertinent information about your medical and surgical history
  • Registration and history forms – available through the clinic website
How do I get refills for my prescription?

It is our policy to fill routine prescriptions during scheduled office visits. It is our intention to provide you with ample medication until you need to return for your next visit. Please contact your pharmacist for refill, allow 48 to 72 hours for refill request.

Narcotics, pain medications and sleeping aids cannot be refilled over the phone.

I think I have an infection. Can you call in an antibiotic for me?

There are many types of infections that need various types of treatments. Diagnosing and treating patients over the phone does not give you the high level of care you and your family deserve. We will be happy to work you into our schedule right away so you can have timely, appropriate medical care. Antibiotics that are not taken properly or are taken for non-bacterial infections lead to antibiotic resistance and “superbugs” that are becoming a serious medical problem in this country.

How can I get my other doctor to transfer my records to Dr. Shu?

We don’t accept other records until you are an established patient. We PREFER that you release the records to YOURSELF and bring them in. Often we don’t need the entire chart, and we will make copies of the pertinent information necessary to complete your medical record.

How can I get my medical records transferred to another doctor’s office?

Send a signed release for medical records with an address or fax number along with a $25 processing fee and we will take care of the transfer.

How can I get a copy of my medical records from One Stop Medical Center?

Signed authorization is required for all requests to transfer medical records. This can be accomplished:

  • In person, at Procedure Clinic

  • By fax. Our fax number is 407-637-2334
Where do the patients have blood work and imaging tests?

All labs and imaging tests are conveniently done in the same building.

When will I hear about my lab and X-ray results?

We usually get the faxed results in a few days. Some results take longer depending on what was ordered. The biopsy results take at least one week. No news is good news, but if you want to hear one way or another, contact the office. Minor abnormalities are discussed at the follow-up appointment.

How can I get a copy of my lab results?

We are happy to provide you with whatever documents you need. Either drop by the office anytime we’re open, or call us and provide us with a secure fax # or we can mail to you.

What are the benefits of office procedures?

Dr. Shu performs the high quality procedures in his office. Our patients benefit from having procedures performed in the familiar, and often less-expensive, environment. In contrast to procedures done in an inpatient setting, there is no facility fee for office procedures. Continuity of care is enhanced. Our patients also benefit from earlier diagnosis and treatments and using their precious time more efficiently.

How can I get a REFERRAL TO A SPECIALIST ?

Our policy is that an office visit is required for all new referrals to a physician or for medical conditions for which you have not been seen in our office. Our referral coordinator will be glad to answer your questions by phone or e-mail. Retroactive referral authorizations will not be made.

How is the provider paid?

Payment is expected at the time of service; we accept cash, checks, VISA, Master Card. Most patients have insurance and payments are obtained by billing the contracted insurance. Appropriate patient portions are collected at each visit.

What is your policy regarding confidentiality of my medical information?

We follow all federal HIPAA guidelines for confidentiality of your health information. All patients are given a copy of these policies. If you need another copy, please ask our office staff.
